Application and Health forms for Silver Lake Band Camp

To apply for Band Camp, follow the below directions. 

 
 

 

INSTRUCTIONS: Please read carefully. 
 

Application Fees for the 2008 Camp:
Total Camp Fees: $540 Accepted by check, PayPal and money order.
Pay using Pay Pal >
$556.20

Press Pay Now button to check out and pay. There will be an additional processing fee of $16.20 for payment via PayPal. You will receive your receipt via email once done with check out. PayPal accepts most major credit cards and you don't need an account with them.
Installment Fees: $180 Accepted by check or money order only.
Payment #1 due with application. 
Payment #2 due May 15th.
Payment #3 due June 15th.

 


Step # 1. Click the below link to download the 
PERMISSION & PAYMENT FORM, fill out, sign,
and return with the application and health form mail to:

Step #2. Click the below link to fill out the Application Form (online). After filling out this form press the send button. Once you press the send button you will be directed to another page which will display your form results of the content you submitted". Once you see the results of your form, print out that page, close the browser window, and return here to repeat these same steps in Step # 3 below.

Step #3. Complete the Health Record Form (online). The procedure is the same as in Step #2.

Step #4. Mail all three (3) forms including your payment or PayPal receipt to:
