Application and Permission forms for Silver Lake Band Camp

To apply for Band Camp, follow the below directions. 

 
 

 

INSTRUCTIONS: Please read carefully. 
 

Application Fees for the 2011 Camp:
Total Camp Fees: $645 Accepted by check, or money order.
Pay using PayPal: $665

Press Pay Now button to check out (with Payment in Full accepted only).There will be an additional processing fee of $20 for payment by PayPal. You will receive your receipt via email once done with check out. PayPal accepts most major credit cards and you don’t need an account with them.

Installment Fees: $215 Accepted by check or money order only.
Payment #1 $215 due with application. 
Payment #2 $215 due May 10th.
Payment #3 $215 due June 10th.

 


Step # 1. Click the below link to download the PERMISSION & PAYMENT FORM, fill out, sign, and return with a copy of the Application Form and check, money order or PayPal receipt.

Step #2. Click the below link to fill out the Application Form (online). After filling out this form press the send button. When you push "Submit", a copy of the form will also be sent to your email so you can print and mail with your payment." An electronic copy will also be sent to our office.
